Cagrilinitide is a long-acting acylated amylin analog designed for sustained receptor activation. Amylin is a 37-amino acid peptide hormone co-secreted with insulin from pancreatic beta cells, and plays a complementary role in postprandial glucose regulation. Cagrilinitide’s structural modifications — including acylation with a fatty acid chain — significantly extend its pharmacokinetic profile compared to native amylin.

Mechanism of Action

Cagrilinitide activates amylin receptors (AMY1, AMY2, AMY3), which are heterodimeric complexes of the calcitonin receptor (CTR) with receptor activity-modifying proteins (RAMPs). Activation of these receptors in the area postrema and hypothalamus modulates appetite signaling, slows gastric emptying, and suppresses postprandial glucagon secretion.
